Box 1: Life Gem
Where you first enter the lab, look to the right after lifting open the door and there will be a box in a small room. Use mist form to go through the bars to reach it.
Box 2: Life Gem
When you return to the present, go back to the room where the two large guards were patrolling and there will be a room on the bottom floor that is now open that you can enter to get this box.
Box 3: Chaos Gem
Just above the previous box, you’ll be able to grab this one on the upper catwalk by jumping to it with the Demonic Wings.
Box 4: Void Gem
On the catwalk just below the previous box, go to the stairs and there will be a gate on the far side. Use the mist form to get through and grab the box on the right.
Box 5: Life Gem
Down below the previous box, there are two grates on the wall that you will need to run through as a rat to get this box. Turn into a rat and then run through the right most grate to chew a wire and turn off a fan. Then exit and enter the left grate. Hop over the wires and run through the fan to enter a room where the box is located.
Box 6: Void Gem
When you enter the round lab where you first run into Raisa Volkova, go to the lab on the right and jump on the structure in the centre. Hang from it and jump to the machinery behind you and then shimmy along the wall to the platform where this box is resting.
Box 7: Life Gem
When you are a rat and there is a guard blocking the large door, turn back human and hit the red light on the upper left with a dagger. Turn back into a rat and go back into the grate you came out of. Follow the passage back to the room with the electrified floor and go through the other vent. Now there will be blocks moving up and down. Go down the left path to find a wire you need to gnaw to turn off the power. Then go back to the right passage past the moving block and you will enter the power room. Turn back human and the pain box is yours.
Box 8: Life Gem
Just before you hop over the electric fence to enter the run-down hospital, go to the left of the large fountain and you will see a gate there. Use mist form to go through and the pain box will be to the left.
Box 9: Chaos Gem
After you open the door in the run down building’s basement by turning the valve and pulling the lever, go to the end of the hall and climb up the rubble to the upper floor. At the end of the hallway will be this box.
Box 10: Life Gem
When you are in the run down hospital, just before you reach the exit elevator, there will be an open room on the right side that contains this box.
Box 11: Void Gem
When you are in the hospital, go to the main staircase area and you will need to double jump your way up to the third floor with the Demonic Wings where the pain box is.
Box 12: Chaos Gem
After you pass through the frozen decontamination chamber where you need to use a chaos bomb, drop down to the lower catwalk and this pain box will be there.
Box 13: Chaos Gem
When you are running through the underground catwalk system, towards the exit, take the left path and you will find this pain box on the other side of a small jump.
Box 14: Chaos Gem
When you climb up the large yellow pipe to get outside to the circular platform just outside the map room, you can find this box on the platform just under the top.
Box 15: Void Gem
After you cross the bridge when you are back outside and reach the HUGE statue in front of the two sets of stairs on either side of the building, go up the stairs to find the box there.
Box 16: Life Gem
When you reach the train station after the big crash, you can run back along the tracks to the previous area where you can grab this at the very far end of the tunnel.
Box 17: Life Gem
Once at the station--after the cargo container ride and the decontamination doors--where there are tracks and there is a fenced in area to the right where the heavy guards patrolled, climb up the ladder to the right and then hop over to the broken ladder. Go up to the platforms and walk to the right a few steps. Turn and look to the right and you will see an opening there with the pain box in it.
Box 18: Void Gem
Across the tracks from the previous pain box in the tunnel, go to the right and you will see this box on the far side on a high platform. Double jump up when you have the Demonic Wings to reach it.
