Box 2: Chaos Gem
After the Lieutenant leaves you, hop up on the metal bars on the pillars and work your way up to the top of the room. On the left side there will be two pain boxes, one across from you and one just below you.

Box 4: Void Gem
After you have defeated Carmilla and are back in the Victory Plaza, turn in to a rat using the shadows to the left of where the Riot Police drops a mine. Go through the grate and drop down the first grate you come to. Follow it to the end and then regain human form to grab this box that will be in front of you.

Box 5: Chaos Gem
Once you deal with the Riot Police in the city square, where the large fountain is, and you make your way over the broken wall to the right, enter the small area on the right side where the map room is to find this in the corner.

Box 6: Chaos Gem
Just after you drop down the vent in mist form and ride the elevator in the garage up to the next room, go through the bars on the right and follow the stairs down to find this pain box there.

Box 7: Void Gem
From the first floor in the large room with two elevator, this box can be grabbed simply be running over to the small staircase at the far end it is resting on.

Box 8: Life Gem
When you are in the same large room, go up the second large elevator and head to the right. Hop across the broken platforms at the end and then jump over to the staircase to reach this.

Box 9: Void Gem
When you are in the large room with the two elevators that you need to power on, ride them to the third floor and then exit to the left. Double jump over the large gap and then you will need to jump to the ladder in the elevator shaft. You will need the Demonic Wings for this, to make the jump. Climb up to the top of the elevator shaft and then double jump again across to the platform where the pain box is.

Box 10: Void Gem
This pain box can be found to the left of the main path during the Antidote II chapter. Before you cross to fight the brotherhood of light, go to the left and you will find this just before the map room.

Box 11: Void Gem
After you have received the Demonic Wings, go to the statue of the lady holding the sword in front of the weapons manufacturer’s building and to the right will be a crashed truck. Hop on it and then hop over the wall to find this pain box.

Box 12: Life Gem
When you are en route to the weapons manufacturer’s during the Second Acolyte mission, hop over the broken wall in front of the building and go to the left side of the entrance to find this box there.

Box 13: Chaos Gem
Once you have defeated the second Acolyte, you can return to the room where you met him during the Revelations chapter. When you take the elevator back up to where he was, look to the right of his room and there will be a hole in the wall that you can jump to grab the pain box.
