Can be purchased from Felkin the Outcast. He's found in the tunnel from Majula to Huntsman's Copse.

Scraps of Life
This is near the Black Gulch Mouth bonfire in Black Gulch. A bit down the path is a giant worm and some old pots. Take him out, then bust up the pots to find this hex atop a corpse.
Can also be purchased from Cromwell the Pardonoer at the Brightstone Cove Tseldora chapel.
Twisted Barricade
This is found at the Doors of Pharros. From the Gyrm's Respite bonfire, head back to the watery room with all the Pharros' doors. Head back further to find a stone ladder you can climb. Up here is a corpse holding the hex.
Whisper of Despair
