 Bunker Triglav 

Enter the area and head towards the command tower.

Enter the domed building next to it where part of the story mission took place.

Stick to the left and you'll spot a doorway. Duck into here.

In this room, go trough the doorway on the right. 

The medallion is on the floor in here.
